Disclosure Statement

 

You have received a Husky Card as your official University of Washington identification. It is the property of the University of Washington and is not transferable. Students are subject to Misuse Penalty WAC 478-120 Student Conduct Code. This card entitles you to establish a Husky Card Account, which is a debit account that allows you to pay for campus services such as food services, printing at computer labs, and parking in the Montlake lot. You can access the complete list of locations accepting Husky Card Account on the Web at http://hfs.washington.edu/husky_card/places.aspx. You may open your account with any dollar amount. By electing to use this account you agree to the terms and conditions of this statement.

 

Lost or Stolen Cards

You must inform the Husky Card Account & ID Center (Husky Card Office) at once if you believe your Husky Card has been lost or stolen. The best way to report a lost or stolen card is through the Husky Card Account Online Office at https://huskycard.hfs.washington.edu, where you can suspend activity on your account until the card is found or replaced. You must inform the Husky Card Office within two (2) business days after you learn of the loss or theft, and your loss will be no more than $50 if someone uses your card without your permission. If you do not inform the Husky Card Office within two (2) business days after you learn of the loss or theft of your card, and it can be proven that proper notification could have prevented someone from using your card without your permission, you could be responsible for as much as $500 in withdrawals. In either case, your loss is limited to the amount you have on deposit.

 

Updating Your Personal Information

To update your address or contact information go to MyUW at http://myuw.washington.edu/ and locate the Change of Address link under the Student Personal Services section.

 

Unauthorized Transactions

Each time you use your card, at any location, your balance is displayed. If you believe that your balance is incorrect, notify the Husky Card Office at once and you will be provided with a statement of your account. If the statement shows transactions you did not make, contact the Husky Card Office as soon as possible. If you fail to notify the Husky Card Office within sixty (60) days after the statement was mailed or given to you, and it can be proven that properly informing the Husky Card Office would have prevented further unapproved use of the card, the money lost after the sixty (60) days may not be credited to your account. If a valid reason, such as a long trip or hospital stay, kept you from notifying the Husky Card Office, the time period may be extended.

 

Documentation

You can obtain a receipt any time you make a deposit to your account or use your account at one of the Husky Card Account point-of-sale terminals (except for self-service terminals in the parking lot and in the laundry rooms). Statements of account will be furnished upon request. You may also go online to view up to thirty (30) days of transaction history.

 

Error Resolution

In case of errors or questions about your Husky Card Account, contact the Husky Card Office. If you think your receipt or account balance is incorrect, or if you need more information about a transaction listed on a receipt or a statement you have requested, notify the Husky Card Office as soon as possible. Note: the Husky Card Office needs to hear from you no later than sixty (60) days after having mailed or given you the statement on which the problem or error appeared.

 

In any correspondence regarding your Husky Card Account:

1. Include your name and account number (student or employee ID number).

2. Describe the error or the transaction you are unsure about, and explain as clearly as you can why you believe it is an error or why you need more information.

3. Indicate the dollar amount of the suspected error. If you notify the Husky Card Office in person or by phone of an error, you may be required to send your complaint or question in writing within ten (10) business days. You will be informed of the results of the investigation within ten (10) business days after you contact the office. In most circumstances any error will promptly be corrected after the investigation. If more time is needed, however, up to ninety (90) days are allowed to investigate your complaint or question. In that case, your account will be credited within twenty (20) business days for the amount you think is in error, allowing you the use of the money during the time required to complete the investigation. Under certain circumstances, $50 may be retained by the Husky Card Office if there is a reasonable belief that an unauthorized transfer of funds has occurred. You will be informed of this credit within two (2) days. You will be notified of its permanency once the investigation is completed. If you are asked to put your complaint or question in writing and the information is not received within ten (10) business days, your account may not be credited.

 

If the Husky Card Office decides there was no error, you will receive a written explanation within three (3) business days after the completion of the investigation. You may ask for copies of the documents used in the investigation.

 

Privacy Policy

Unless otherwise required by law, the Husky Card Office may disclose information to third parties about your account or the transfers you make, only in the following circumstances:

1. Where it is necessary for completing transfers; or,

2. In order to verify the existence and condition of your account for a third party, such as a merchant; or,

3. If you give the Husky Card Office your written permission; or,

4. In order to comply with government agency or court orders or to respond to any other compliance requirement.

 

Limitations

Accounts inactive for one year may be closed. Inactive Husky Card Account balances will be held by the University of Washington for at least two (2) years. During this period, cardholders may request a refund of their balance, or reopen their account by contacting the Husky Card Office. After two (2) years, balances held will be turned over to the state of Washington as unclaimed property. Unclaimed property can be retrieved indefinitely by contacting the University of Washington General Accounting Office at 206-523-8472. There are no limits on the number of transactions on your account. However, the dollar amount of your transactions may not exceed your current account balance.

 

Refunds

Husky Card balances may be refunded upon written request. The request must include your name and account number (student or employee ID number), along with a current mailing address. You can stop by the Husky Card Account & ID Center, located on the ground floor of Odegaard Undergraduate Library, and complete a refund form, or send us an email from your UW email account. We can be reached at huskycrd@u.washington.edu. All refunds are processed by check or the credit card used and will be issued within ten (10) business days of receipt of the written request. Accounts less than $5.00 may be charged a service fee, up to the total amount of the existing balance.  Please note: we do not permit cash refunds or withdrawals.
